After Hours Care

When Shalom Health Care Center is closed:

A Provider is available for urgent medical needs after our Primary Care Clinics are closed. To reach the on-call Provider to address these urgent needs, please call our main line number at 317-291-7422 to be connected with our after-hours messaging center.

Please allow time for someone to return your call and be available when they call you back. Please keep in mind that your own personal Provider may not be the Provider on-call at that time and you may receive a call from another Shalom Health Care Center Provider. However, all of Shalom’s Providers will be able to access your full medical record to provide you with the best medical advice. You will be asked to provide the following information:

  • Patient’s name and date of birth
  • Short description of medical issue you are experiencing
  • Provider’s name
  • Names of all medications you are taking, including dosage amounts
  • A 24-hour pharmacy telephone number
