Shalom Health Care Center is pleased to announce that it has been awarded Patient-Centered Medical Home (PCMH) Recognition by the National Committee for Quality Assurance PCMH Program. PCMH standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication, and patient involvement.

O’Kane. “NCQA’s PCMH Recognition shows that Shalom Health Care Center has tools, systems and resources to provide their patients with the right care at the right time.”
