Leonardo Ortega, M.D., M.P.H.
Dr. Ortega has over 30 years of experience in management of health care delivery systems and public health management. He is a trained Physician from Central University of Venezuela and holds a Masters Degree in Public Health from Johns Hopkins University School of Public Health. He was appointed as Shalom Health Care Center CEO in 2012. Dr. Ortega is fluent in Spanish and English.

Margarita Wiersema, M.D.
Dr. Wiersema is a board-certified family practice physician. She has more than 20 years of clinic expertise since graduating Medical School in the former USSR. She previously treated patients at Community Health Medical Center; and also served as a physician with the United States Army, receiving the Meritorious Service medal for her care of troops. Dr. Wiersema treats patients of all ages and has special interest in diabetes, heart disease prevention, women’s health, and medical procedures. She fluently speaks English and Russian.
Mimi Beresky
Mimi Beresky joined Shalom in 2010 and has held a variety of positions in front desk, scheduling, insurance navigation and Community Outreach. She was appointed to the Chief Operations Officer at Shalom in 2015 and was instrumental in the opening of the 56th St Clinic in 2016. She is fluent in English and Spanish and has worked with a diverse patient and employee population.
Andy Nino
Michael “Andy” Niño joined Shalom in 2017 after serving eight years on the Board of Directors. He has extensive experience in the banking industry, most recently as Assistant Vice President Branch Manager with BMO Harris Bank. Andy is active in the community, serving on the boards of Fuller Center of Housing and Indiana Latino Expo. He is fluent in English and Spanish.
Staci Walters, MPH
Staci Walters holds a Master’s Degree in Public Health. She joined Shalom in 2016 as QAQI Associate and advanced to QAQI Director. Her academic research in medical underserved areas and health professional recruitment and retention developed her passion for serving vulnerable populations.
Morella Dominguez
Before joining Shalom in 2014, Morella served as Director of Multicultural Affairs at the Indiana Minority Health Coalition. She also served as Spanish/English Medical Interpreter at Wishard Health Services and Director of Production for the television program”Nuestra Musica” at Butler University. She has a BS in Telecommunications Arts from Butler University.
Jessie Combs, MSN, APRN, FNP-C
Jessie Combs is a Family Nurse Practitioner and has worked in pediatrics since 2012. She joined the Shalom School-Based team in 2018, taking the position of Team Lead in 2022, and then becoming Program Director in 2024. Jessie’s passion and focus is to provide healthcare for the underserved children in our city and to ensure every child that comes into our clinics knows they are cared for and loved. Outside of Shalom, Jessie enjoys spending time with her family, doing Peloton workouts, hunting, and attending Traders Point Christian Church.
Fred Milch
Fred joined the team in March 2023. Mr. Milch graduated from Indiana University with a Bachelor’s Degree in Computer Information Systems and has held various certifications over the years, including Microsoft Certified Systems Engineer and Zultys Certified Systems Engineer. He has been married for 24 years, has two human children and five furry ones. He enjoys games of all genres, frisbee golf and home brewing. His diverse technical skills include Microsoft servers, networking, telephony, Exchange administration, as well as Office 365 and Azure AD administration. Prior to working at Shalom, he worked in Information Technology for twenty-four years, seventeen of which were as a level 3 analyst overseeing a team of system engineers. Mr. Milch began his career as a COBOL programmer for Ameritech’s billing department. He then moved into desktop and server support, eventually overseeing IT support for SBC Communications for the state of Indiana. From there he moved into a senior analyst position for SIM2K Technology Consultants supporting environments in a wide variety of industries including manufacturing, engineering, health care and real estate. At Shalom, he ensures the operation of the company’s networks, servers, business applications, telephones, and helpdesk while overseeing the IT team.

Dr. Lyle Cortez, D.O.
Dr. Lyle Cortez was born in Chicago, IL but grew up in Terre Haute, Indiana. He earned his Bachelor of Science Degree from Indiana University in Bloomington, Indiana. He completed medical school at Midwestern University’s Chicago College of Osteopathic Medicine in Downers Grove, IL. He then completed his Family Medicine Residency at Indiana University’s School of Medicine in Indianapolis as part of the Class of 2012. Dr. Cortez is Board Certified in Family Medicine by the American Board of Family Medicine.
Being a D.O., he has been trained in Osteopathic Manipulation Treatment. It is similar to Chiropractic Adjustments. Dr. Cortez takes pride in listening to his patient’s concerns and needs. With his training in rural and urban areas, he is confident in helping people of different cultures. He strives to give the best care possible, and if he needs assistance, he is not reluctant to place a referral to a specialist. He is a true believer in Preventative Medicine by doing Annual Visits. He is also passionate about treating Diabetes, Hypertension, and High Cholesterol.
He comes from a strong family background of medical providers, and being a doctor was something he has always strived to be.
Dr. Hemalatha Hemachandra, M.D.
Dr. Hemachandra has been a Pediatrician for over 25 years, working previously in Neonatal Intensive Care and as an Oncology Specialist. She has worked in large pediatric practices and with various ethnic populations. In addition to working in Indianapolis, she has worked in Greenfield (IN), Muncie (IN), Michigan, Washington DC, and Maryland. She is passionate about being an advocate for children and educating their parents. Dr. Hemachandra believes that reading from an early age can change the direction of a child’s thinking and future achievements and that children should always come firsts. She enjoys spending time with her three children and two grandchildren, gardening, and reading poetry.
Tyler Trabel, PA-C
Tyler is a Physician Assistant from Fort Wayne, IN, who started with Shalom in 2020. He received his Bachelor of Science in Biology/Pre-Physician Assistant from Ball State University and his Master of Science in Physician Assistant Studies from the University of Saint Francis (Fort Wayne). Tyler has a background in emergency medicine but is looking forward to establishing himself in primary care and developing relationships with his patients to help combat acute and chronic conditions/diseases. He prides himself on pushing his patients to be the best that they can be.
Tyler enjoys watching and playing sports in his free time, spending time with family and friends, and traveling with his soon-to-be wife.
Lyndsey Furry, FNP-C
Lyndsey joined Shalom in 2015 as a family nurse practitioner, treating acute and chronic conditions in children and adults. Surviving cancer in her teenage years inspired her passion for serving others and focusing on chronic disease prevention and management through lifestyle modification and access to affordable screenings, medication, and care. Valuing a holistic approach, she seeks to care for her patients as whole, individual people.
Outside of Shalom, Lyndsey enjoys sewing and crafting, playing the ukulele, and spending time with her husband and her dog, Neptune.
Lori Fishburn, MSN, APRN, FNP-C, PMHNP-BC
Lori received a Bachelor of Science in Nursing Degree from Indiana University and a Master of Science in Nursing from Indiana Wesleyan University in 2016. She is a University of Saint Francis student working on a Doctorate in Nursing Practice. Lori has over 20 years of experience in health care, working in many nursing specialties, including ER, ICU, and Occupational Health. Lori also specializes in working with people seeking substance use disorder treatment through Medication Assisted Treatment.
She is passionate about diversity and acceptance in health care and improving access to care for people through inclusivity. Lori enjoys spending time with her husband, daughter, and two dogs. She also enjoys reading, sports, and music.
Dr. Galstian is a board-certified physician in Obstetrics & Gynecology. He has practiced for over 38 years in three countries. Over the last 26 years, he has worked in Indianapolis, first as a Methodist Hospital employee and then for 24 years as a solo practitioner. Dr. Galstian was awarded a Ph.D. for his studies in the field of prematurity and fetal growth restriction. He co-authors the “Immunology of Preeclampsia” chapter in the textbook “Immunological Obstetrics.” He has consistently been named as one of Indiana’s Top Doctors since 2011. He has also been a clinical preceptor for OB-GYN and family practice residents. Dr. Galstian welcomes all patients and performs all surgical procedures pertinent to general Obstetrics and Gynecology. He is fluent in English, Russian, and Armenian.
